Your
first visit
Your
first visit will consist of a consultation that will include
a complete examination and review of your personal and medical
history. Often this may require the use of an X-ray or other
specialized diagnostic tools. This information will allow
us to provide you with a detailed explanation of your diagnosis
and your available treatment options. Our job is to provide
you with the required information so that you can make an
informed and educated decision in regards to your personalized
treatment.
For
patients who are under the age of 18...
A
parent or guardian will be required to accompany you on
your visit.
Your
appointment is a time that has been especially reserved
for you. If for some reason you are unable to keep your
appointment, please contact our office so that we may make
this time available to one of our other valued patients.
Your
Medical History is Important
We Need to Know!

pressure, diabetes, rheumatic fever, heart valves, prosthetic
joint or appliances, etc.)
-
Current
and past medical and dental treatments (i.e. surgery,
dental or medical treatment, etc.)

If
you are a referred patient
Please
remember to bring any referral slips and X-rays that your

us in helping you with your visit.
Please
remember if your X-rays are older than one year or are not
sufficient to diagnose the problem in question, we may need
to take new ones.
