Your First Visit

Your first visit will consist of a consultation that will include a complete examination and review of your personal and medical history. Often this may require the use of an X-ray or other specialized diagnostic tools. This information will allow us to provide you with a detailed explanation of your diagnosis and your available treatment options. Our job is to provide you with the required information so that you can make an informed and educated decision in regards to your personalized treatment.

For patients who are under the age of 18…

A parent or guardian will be required to accompany you on your visit.

Your appointment is a time that has been especially reserved for you. If for some reason you are unable to keep your appointment, please contact our office so that we may make this time available to one of our other valued patients. Please note that appointment times are tailored to each individual patient and time will be reserved for YOUR appointment. Our office does require a 48 hour notification for ANY cancellations or rescheduling whether they are consultations or scheduled procedures.  Failure to provide proper notice of cancellation may require a prepaid portion of the charges to reschedule.

Your Medical History is Important…We Need to Know!

A complete review of your medical history is important and may alter your treatment. Please provide us with all information regarding…

Current and past medical conditions (i.e. asthma, high blood pressure, diabetes, rheumatic fever, heart valves, prosthetic joint or appliances, etc.)

Current and past medical and dental treatments (i.e. surgery, dental or medical treatment, etc.)

Any medications or herbs that you may be taking (i.e. aspirin, blood thinners, inhalers, blood pressure medication, over-the-counter medicines, etc.) 

PLEASE BRING A CURRENT LIST OF ANY AND ALL MEDICATION, SUPPLEMENTS and HERBS WITH YOU TO YOUR APPOINTMENT.

Allergies that you may have to any medications (i.e. penicillin, codeine, aspirin, latex, etc.)

NO REFERRAL IS NECESSARY TO BE SEEN IN OUR OFFICE.  HOWEVER, If you are a referred patient…

Please remember to bring any referral slips and X-rays that your dentist or physician may have given to you so that care may be coordinated as per your dentist’s and your wishes.  This will aid us in helping you with your visit.

Please remember if your X-rays are older than six months or are not sufficient to diagnose the problem in question, we may need to take new ones… especially if treatment is more complicated (for example: extensive nerve involvement)
